Primary Care & Wellness Assessments
We know it isn’t always easy to go see a doctor. There are times when travelling to an appointment is an inconvenience or even an impossibility for some patients. That’s where Iris Health comes in. Our exemplary team of providers – including physicians, nurse practitioners, and physician assistants – brings the medical care right to your home. Not only do House Calls reduce the hassle of seeing your doctor at a medical office, but it allows our team to provide even more personalized, comprehensive care in the comfort of your own home.
Transitional Care Management (TCM)
Transitional Care Management makes it possible for Medicare patients to receive oversight and support of their medical care during the transition between an inpatient setting, such as a hospital or skilled nursing facility, and their home setting. Our team serves patients and their caregivers during this transitional period by communicating on any new or existing medical concerns, educating on effective self management, and facilitating access to additional care such as home health or community services. We keep our number one goal in mind during the entire process, that there be no gaps in proper medical care for any patient.
Remote Patient Monitoring (RPM)
RPM is a Medicare-covered service that is designed to help you monitor and manage your health in the comfort of your own home. It also keeps your health care provider informed, in real time. After reviewing your information one of our medical professionals will order the appropriate equipment to be delivered to your home. Once it arrives you can start monitoring right away. Iris Health provides cellular monitoring equipment such as blood pressure cuffs, glucose monitors, pulse oximeters and digital scales that you as a patient uses remotely, in your home. The RPM equipment will collect the data and automatically relay your healthcare information to the Iris Health Control Center platform.
Chronic Care Management (CCM)
Chronic Care Management provides oversight of care of Medicare patients who have two or more chronic conditions expected to last at least 12 months. Our practitioners can establish, monitor and maintain care plans for those who would strongly benefit from continuous care. This ongoing relationship with our patients and their caregivers ensures that there is support to reach an individual’s health goals, access to health information, and necessary primary and preventive care.
Advanced In-home Wound Care
Our team understands the obstacles some face with transportation to a wound clinic that is why we bring it to you at home. Using advanced wound care products coupled with an individualized care plan. We offer a variety of treatment options for difficult or non-healing wounds. Your treatment plan is designed to get you on track to manage and heal your wounds.
Telehealth Services
Sometimes there are obstacles to coming into a medical office, but help is still available. Our providers can meet you over a HIPAA compliant connection. Same day appointments available!
